Certainly this is one of the strongest points against system wide menubars (either the Mac or NeXT style) - they completely, absolutely kills focus follows mouse.
Since I was brought up on Macintosh System 6/7, however, I'm very much used to this menubar flavor, and I feel it's more elegant than having menus in windows. One reason is that it makes it feasible to have applets in the menubar, another is that it doesn't take up space in every window, and finally there's the inevitable Fitts' law.
Even more elegant, though, is to get rid of the menubar all together by only using right-click menus. This is one of the distinguishing aspects of RiscOS. To get a taste of this concept, you needn't look any further than ROX-Filer (Risc On X).

Seems like this would break the focus follows mouse option which most older X users like best. MacOS doesn't have that option. That alone would be reason enough to just say no.
